Introduction

Daniel Seddon Psychology ("we", "us", "I", "our", "me") is committed to acting in accordance with the Personal Health Information Protection Act (PHIPA, 2004) as well as the Personal Information Protection and Electronic Documents Act (PIPEDA, 2011). The following policy explains how your information is used, collected, and stored while using this website and/or engaging in my services.

What is personal health information?

Personal health information (PHI) is information about an identifiable individual. Examples of personal health information may include:

  • Details regarding the physical or mental health of an individual.

  • Full name.

  • Date of birth.

  • Contact information.

Why do we collect personal health information?

We collect personal health information for the purposes of providing services to our clients. The primary purpose of collecting personal health information is to inform service delivery to our clients. For example, we we may collect names, information about mental health history and physical problems to render professional opinions and inform psychotherapeutic approaches. A secondary reason for collecting this information is to monitor changes to the individuals progress over time. We also may collect, use, and disclose information for purposes related to or secondary to our primary purposes. The most common examples of our related and secondary purposes are as follows:

  • To obtain payments for services provided.

  • To comply with external regulators (more information is provided on this in the "disclosure of information" section.

What happens if there is a privacy breach?

Many precautions are taken to limit the risk of a privacy breach. However, in the unlikely event this does occur, you will be notified. Upon learning of a possible or known breach, we will take the following steps:

  • Contain the breach to the best of our ability.

  • Take steps to prevent unauthorized access to electronic information (e.g., change passwords, restrict access, temporarily shut down systems).

  • We will notify impacted individuals and provide current contact information in case there are further questions.

  • We will provide the Commissioners' contact information and advise the individual of their right to complain to the Commissioner. 

  • An investigation will be conducted to determine what can be done to limit further or future breaches.

Depending on the circumstances of the breach, we may notify and work with the Privacy Commissioner of Ontario.

This information is collected for the purposes of understanding how visitors interact with the website to potentially inform changes to content or website experience.

If you choose to volunteer personal information through the form on this website, this information will be collected and stored. Information offered through this form may be considered personal health information as it may include your name, as well as particular information about your mental health concerns. By submitting this form you understand that despite extensive security measures, electronic communications can be intercepted or modified without the sender or recipient's knowledge. It is advised that you bear this possibility in mind when considering what information you are including in this form. If you choose to submit a contact form, you are consenting to these potential risks.

Disclosure of information

Information shared with Daniel Seddon Psychology is generally considered confidential with the following exceptions: a client intends to cause serious physical harm to themselves or another person, a disclosure is made about sexual abuse from another regulated health professional, your clinical record is subpoenaed by court order, I suspect a child or a member of a long term care facility is being harmed or abused, or for quality assurance purposes by request of the College of Psychologists and Behavioural Analysts of Ontario.

When using this website, cookies may be installed on your computer which track your preferences and behaviour. Users may control the way cookies are used by responding to the banner that pops up upon entering the landing page on the website. Alternatively, cookie settings can be adjusted in your browsers settings. Please note that adjusting cookies may have an impact on overall website functionality. 

Therapeutic Relationship

Please note that submitting an inquiry to Daniel Seddon Psychology does not make you a client of the practice. You are only considered to be a client of the practice if both Daniel Seddon and yourself consent to the initiation of services.

Third Party Professionals/Contractors

It is possible that through the delivery of services, limited information about you may be accessible by other third party professionals, agencies, contractors, etc. Examples of these professionals may include lawyers, accountants, credit card companies, computer support specialists, etc. Every effort is made to restrict access to personal information as much as is reasonably possible while these parties carry out their relevant duties.

Advertising

Information collected by means of cookies or through Wix Analytics may be used for the purposes of improving this website or for targeting advertising. 

Updates

Record Keeping Duration

As required by PHIPA and PIPEDA, records are kept for 10 years from the date of last contact and for 10 years after a minor turns 18.

Accessing Information

With few exceptions,  you are entitled to know what information I have about you on record. A simple request for this information is often all that is necessary to obtain it. If you believe there is a mistake in your record, you have the right to have it corrected. Please note that this applies to information that is factually incorrect and does not apply to professional opinions rendered about you. If we do not agree with your opinion that a mistake has been made, a notation will be made regarding your statement to the matter. We reserve the right to respond to these requests within 30 days and to charge a reasonable fee to release the requested records.

Artificial Intelligence (AI)

Artificial Intelligence is not used in any way shape or form in Daniel Seddon Psychology's practice or operations.
